New Patient Paperwork

• Please complete any paperwork you were sent in advance of your first appointment and bring a copy with you to your first appointment.

Insurance Cards

• Please bring your insurance card, photo identification and any copayments to your visit.

  • Accepted insurance plans may change.

  • Once referred, my biller will look at benefits and authorization issues to see if you will be covered.

  • Dr. Alfano currently accepts the following insurances:  Medicare, Sansum, Blue Shield, Magellan, Anthem Blue Cross.  *Subject to change

 

Other

• Please remember to get a good night's sleep, eat breakfast, bring your glasses, wear your hearing aids (with good batteries), continue to take your prescribed medications as usual.  If you are experiencing bladder control issues, please wear undergarments to the testing exam.
