Your intake paperwork will be sent to you via secure patient portal.  Please complete the required patient paperwork prior to your first visit with Dr. Alfano.  You can scan and upload it into your patient portal or fax it to (805) 765-9557.

Insurance Cards

• Please bring your insurance card, photo identification and any copayments to your visit.

  • Dr. Alfano currently accepts Medicare and select insurance-Practice Manager, Amy Vukovic, will be able to determine whether your insurance can be used.  She can be reached at 805-330-2151

Other

• Please remember to get a good night's sleep, eat breakfast, bring your glasses, wear your hearing aids (with good batteries), and continue to take your prescribed medications as usual.  If you experience bladder control issues it is most important you wear the necessary undergarments to the appointment.
